To provide community cardiac rehabilitation services to patients following myocardial infarction, pre and post revascularisation under the supervision of the specialist cardiac nurses. The role will involve supporting delivery of a comprehensive, structured exercise and education programmes and also working in home & clinic settings. The post holder will work closely with the multi-disciplinary team, health professionals and voluntary sector, concentrating particularly on engaging vulnerable and ‘hard to reach’ groups.
To assess, plan, provide and evaluate the cardiac rehabilitation needs of individuals following myocardial infarction, revascularisation or cardiac surgery under the supervision of the specialist cardiac nurses.
To offer and provide lifestyle advice, support and treatment, assisting the patient in identifying risk factors for CVD and initiating change and referring on to appropriate services as necessary.
Providing psychological support and interventions / escalation as necessary.

1.2 To offer and provide lifestyle advice, support and treatment, assisting the patient in identifying risk factors for CVD and initiating change and referring on to appropriate services as necessary.
1.3 Providing psychological support and interventions / escalation as necessary.
1.4 Be sensitive to the cultural needs of patients from minority ethnic groups, including the use of interpreters as necessary.
1.5. Provide education and support to families and carers ensuring they have access to appropriate information.
1.6 Liaise with community and secondary care colleagues and other professionals as required with regard to the patient’s individual care, medication and other needs.
1.7 Deliver stage’s 0- 5 cardiac rehabilitation according to local guidelines
1.8 Responsible for monitoring patient status during exercise and responding appropriately.
1.9 Using appropriate clinical equipment assess and monitor patients progress and act on abnormal
results.
2.0 Maintain accurate and detailed records in accordance with Nursing and Midwifery
Council Standards.
2.1 Take responsibility for supervision of the health care assistants working within the
service under the guidance of the specialist nurses
