Description:

We are looking for a compassionate, enthusiastic and caring Healthcare Assistant, who is reliable and flexible to join our Community Continence team, covering all of Croydon.
You will be under the partnership with the team members, servicers users, general practice staff and other agencies. You will work as part of the continence nursing service and other stakeholders to provide efficient and effective nursing care to all patients their own home. You will help to support the management of patients who will require long term monitoring of the continence status, to ensure all high standards performed in a timely manner and monitor the appropriate use of containment products.
You will be representing Croydon Continence service professional by displaying a positive approach when promoting patients health, carers and other agencies. With this you will need to have a good interpersonal and organisational skills.

INTERVIEW - TBC

The post holder will work independently under the direction and guidance of a registered nurse in order to provide efficient and effective evidence based nursing care to patients in their own homes. He/she will work in partnership with team members, service users, general practice staff and other agencies.
To work as part of the Continence Nursing Service and other stakeholders to provide efficient and effective nursing care as delegated to patients in their own home.
Perform the initial continence assessment without direct supervision for patients (including housebound requiring a home visit) who are referred to the continence service and patients in residential homes
To liaise closely with the Continence Specialist Nurse (CSN) in devising plans of care for each patient.
To support ongoing management of patients who require long term monitoring of their continence status. Ensure high standards of continence assessments are performed in a timely manner and monitor the appropriate use of containment products.
To represent Croydon’s Continence Service professionally by displaying a positive approach when promoting good bladder and bowel health to patients, carers and other agencies.
