Description:

ESSENTIAL CRITERIA

  • Expert clinical skills in the field of diabetes.
  • Expert knowledge of insulin and GLP-1 receptor agonist therapies.
  • Able to work independently, recognising own limitations and seeking advise and support when required.
  • Able to demonstrate an understanding of the quality and governance agenda and actively participate in evaluation and improvement of quality within the service.
  • Excellent communication skills, verbal and written.
  • IT literate, with an ability to analyse clinical data.

  • Evidence of personal and professional development, reflective practice and supervision.
  • High level of motivation and enthusiasm to deliver a high quality service.
  • Adaptable/flexible to meet the needs of the department / working practices and the wider organisation.

Responsibilities:

MAIN DUTIES OF THE JOB

  • To promote the health and wellbeing of people with complex diabetes enabling them to achieve optimum glycaemia control through informed self management.
  • To empower and support people with diabetes to self-manage their condition, guiding them towards a ‘normal’ lifestyle and reducing morbidity and premature mortality by detecting and preventing complications
  • To provide education and support to both people with diabetes and those who care for them.
  • To promote strong, effective and regular collaborative working between all health care professionals and stakeholders along the diabetes care pathway
  • To provide a link between the GP and consultant led specialist teams.
