Description:
Are you passionate about improving the patient journey from hospital to community? Do you want to progress your leadership skills and be at the forefront of change?
We are seeking a Community Discharge Planning Nurse to work as the pivotal link between Bedfordshire Hospitals and Bedfordshire Community Therapy Health Services.
You will help provide patients and families with the information they need to understand the discharge process on a case by case basis, considering the patient in their most holistic sense. You will be confident in maintaining positive relationships with hospital staff and staff within the community bedded settings as well as the wider multidisciplinary team. You will ask the right questions at the right time to ensure patients are discharged safely on the appropriate pathway whilst upholding the Trust Values.
You will be confident in your ability to support, negotiate and be creative to develop solutions to various challenges relating to individual patient’s needs and wishes. You will be innovative, forward thinking, self-driven with excellent communication and interpersonal skills. You will be an experienced Registered Nurse with experience in discharge planning in either the Acute or Community sector.
- The Discharge Planning Nurse/Therapist will be the point of contact for Bedfordshire Community Services within the acute setting.
- The role is required to deliver high standards of patient care and pathway management to reduce delayed transfers of care and ensure safe and timely transfer of care into appropriate community settings including intermediate care services and bedded units.
- Having extensive knowledge of discharge practices including Discharge to Assess pathways and community services the post holder will be required to identify and facilitate discharge and prevent unnecessary hospital admission.
- The role requires excellent timely and communicative skills to ensure all parties: services, patients, families and carers are informed and educated regarding the appropriate pathway the patient will take on discharge from hospital.
Be the point of contact for the patient/families/Carers 3 working days post discharge.
