Community Forensic IDD/MH Nurse

We are looking for an experienced registered nurse (RLDN) with current registration with the nursing and midwifery Council (NMC). The Assertive Transition Service will work to expedite the safe, smooth discharge of Forensic Learning Disability and Autism patients from secure hospital into the community, by providing earlier support in a persons discharge pathway and more robust care and support post-discharge. The teams will consist of multi-disciplinary health and third sector agency and peer support staff to form one integrated service You will work carrying a complex caseload over Notts, Derbys and Lincs and provide in reach work to secure and community settings to support the transition of individuals leaving hospital. You will have sound knowledge and understanding of working with individuals with a recognised IDD and/or ASD , with a forensic risk.
The importance of working with colleagues from multiple agencies to ensure a robust package of care is available. You will work in partnerships with IMPACT case managers, Transforming Care, Community Forensic Services, Adult Mental Health, Private Sector and Local Authorities. You will work directly with individuals and their families and carer groups. You will deliver person specific interventions inclusive of training packages You will have a sound knowledge of Mental health legislation including MHA, MCA including DoLs
