JOB OVERVIEW

This is an exciting opportunity for an enthusiastic, inspiring and a reliable Community Health Care Assistant, who is willing to support and deliver a range of high quality nursing interventions in the out of hours period.
A commitment to support our team in delivering high quality services is essential for this role, and you will also need to demonstrate excellent communication and interpersonal skills with the ability to be adaptable, trustworthy and approachable.

Part time - 15 hours per week (This will include some weekend and bank holiday working on a flexible rota).
CWP has adopted the Living Wage. Where the standard Agenda for Change hourly rate is below Living Wage, pay will be supplemented to this level.

Responsibilities:

MAIN DUTIES OF THE JOB

Working within our integrated team, you will undertake duties involving the delivery of nursing services using a person centred approach. This involves providing quality care to patients in their own home, or patients in long term care settings, avoiding unnecessary admission to hospital.
As the role involves visiting patients in their own environment, access to transport for work purposes is essential.
