POSITION SUMMARY

The Community Health Representative’s (CHR) primary goals are to improve the quality of life of the Seabird Island Band community members by assisting them to make healthier lifestyle choices; promote client dignity, independence, comfort, mobility, personal appearance, and safety; and to provide support to the Community Health Nurse(s) (CHN) and/or Home and Community Care (HCC) Nurse. These goals are to be achieved by using professional public health knowledge.

QUALIFICATIONS

  • Health Care Assistant or Community Health Representative certificate required.
  • At least 2 years of relevant experience working within First Nations communities, focusing on community health.
  • Standard First Aid and CPR Certification.
  • Ability to provide services in a prompt, respectful, fair, equitable, unbiased, and non-judgmental manner and according to existing policies and procedures.
  • Ability to be accessible, accountable, and responsive to community members. Possess excellent customer service skills.
  • Excellent communication and relationship-building skills.

Responsibilities:

WHAT YOU’LL DO

  • Conduct the initial home visit and refer clients to HCC and/or appropriate service.
  • Conduct short-term individual, family, and group counseling in the field of health.
  • Administer the patient travel services according to the patient travel policies and procedures.
  • Maintain inventories and distribute clinical supplies as required.
  • Set up mobile clinics such as Mammogram, Optometry, and Hearing.
  • Assist in coordinating accommodation for community members traveling distances for health reasons.
  • Identify gaps in services and inform the Home and Community Care Supervisor and the department team, as required.
  • Provide Home Support services including meal preparation, personal care, palliative care, transportation, and transfer of functions as requested by the HCC/CHN Nurse.
  • Provide follow-up visits to monitor the health of clients including pre-natal, hospital discharge and other illnesses as requested by the HCC/CHN Nurse.
  • Record services provided in the client health care records.
  • Maintain professional and accurate client service records.
  • Prepare all necessary documentation for community members as needed.
  • Attend scheduled team meetings with the Health and Social Development department staff and other meetings as requested by the Better at Home Supervisor.
  • Consult with the Better at Home Supervisor on workshops and services; and provide updates on community health needs and concerns.
  • Be familiar with policies and procedures governing the delivery of community health services.
  • Establish and maintain professional relationships with Seabird Island Band employees and representatives from other governmental and private organizations.
  • Keep updated on Medical Services Branch regulations, policies and procedures that affect the delivery of health programs and services.
  • Submit information/educational articles pertaining to health education, awareness, promotion, and prevention for publishing in the Seabird Island newsletter.
  • Participate in case conferences as directed by the Health Services Program manager, or as requested by other Health and Social Services Agencies.
  • Advocate for Seabird Island Band members’ rights and entitlements where necessary.
  • Plan, coordinate and implement the Home and Community Care services and the Health Transfer Agreement.

  • Other duties or responsibilities as assigned by your Supervisor, Manager, Director, Chief Administrative Officer, or Chief & Council.
