MINIMUM QUALIFICATIONS:
- Certification as a Community Health Worker preferred.
- 2 years of relevant experience
- Experience working specifically with individuals living with alcohol use disorders is preferred.
- Demonstrated ability to work with diverse and/or low-income populations, including those experiencing homelessness, unstable housing, mental illness and/or substance use disorders.

Responsibilities:
- Welcome and orient clients to the role of community health worker in the managed alcohol program. Complete health and risk assessments associated with long-term alcohol use. Work with case management staff and client to establish and maintain an alcohol harm reduction plan.
- Collaborate with case management staff in assisting clients to achieve their goals.
- Problem solves with housing staff to mitigate barriers to maintaining stable housing.
- Support plan for follow up care as determined by the resident and their primary care provider.
- Arrange and coordinate needed health care, which for any client may include, acute medical care and home health nursing; primary care; dental care; vision care; therapies and mental health evaluation.
- Maintain contact with care providers in instances of acute care or extended care episodes (cancer care, protracted dental and post-surgical care.)
- Help clients learn how to effectively navigate the health care system.
- Educate clients, on the value of preventive health
- Obtain needed consents.
- Develop rapport with clients and their families (as indicated) ) to ensure follow-through with needed resources.
- Arrange medical transportation if necessary.
- Provide group and or individual health education as directed by program manager.
- Promptly and accurately collect, enter, manage and report client data into systems of record.
- Maintain Contact and act as liaison with the providers of service and/or social service agencies in the community and the metropolitan area regarding client health.
- Other duties as assigned.
