Description:

The Morecambe Bay Heart Failure Team deliver care to patients living with Heart Failure, through Out-patient clinics and Home Visits. As the team administrator you would be the key person to make/amend appointments, answering general questions, take messages for the clinical team and help in arranging the clinics across various basis amongst other duties which are listed within the Job Description. The Team is constantly developing and the role will change as the service does to ensure that the patients receive the best care possible.

The role will involve regular contact with the public, specifically patients, carers, relatives and other health care professionals, in an array of customer service duties, so a warm, friendly and helpful manner is essential.
The role will involve making, changing, cancelling clinic appointments, listening to and signposting service users, passing on messages to clinical staff. Experience of and proven competency in using Microsoft office suit and NHS clinical health record systems (e.g. EMIS Web) are essential.
In this role you will create and send appointment letters, receive and manage referrals, input and record information onto our electronic patient record (EMIS web) and Lorenzo (Inpatient electronic record), photocopy and collate assessment packs, patient information packs and additional information for the Heart Failure Clinics. You will be expected to support our Clinical Team Across the Bay in the collection of service performance data.
You will be a key member of our team, so a friendly, respectful team-player is essential.
UHMBT Integrated Community Service provides care outside of the hospital setting, including community locations and people’s own homes.
Our Hospital Services operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life-threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
Our community services for adults are provided in people’s homes, community centres, clinics, GP Practices, community hospitals and our main hospitals. Our aim is to work with people to help them remain independent, improve their health and manage their conditions through high quality care, advice and support.
