Description:

The purpose of this post is to work alongside the Locality Matron, Home First Bureau and the ward Discharge Facilitator Teams to assess, coordinate and manage referrals for transfer to community beds to optimise flow and ensure care is provided close to home.
Assess suitability for transfer to Community Hospital (including for rehabilitation, palliative care and discharge planning) with the full support of Care of the Elderly (CotE) Team.
Communicate and liaise with a range of partners, both internal to the HB (BCUHB East, Central and West and social care / community partners) and senior managers; and external parties (including across the NHS and Social Care in Wales and England) and contractors.
A bleep must be held at all times for communication purposes.
The post holder must demonstrate and provide leadership qualities to ensure timely referrals, completed assessments and follow up of transfer plans.
Also proactively and effectively liaise and communicate with Managers any managerial or operational problems / decisions or changes in practice that affect the department
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
