Description:

An exciting opportunity has arisen for a Band 5 care coordinator to work in our lively and friendly team based in the multi-cultural city of Bradford. We are looking for an enthusiastic and passionate clinician to join our fantastic Bradford City and North Community Mental Health Team for Older People.
Covering a wide area of our District, we enjoy working within one of the most diverse and culturally rich populations in England. We are always looking for new and innovative ways of delivering care to those with protected characteristics.
Working with our older population is a genuine privilege. It provides many valuable and unique experiences to affect the overall quality of life for a population that can be predisposed to comorbidities, reducing physical health, reducing mobility, and isolation. Providing care in these situations can be uniquely rewarding.

(a) Clinical Strategy

  • Responsible for the delivery of community mental health care to a caseload of clients in accordance with the objectives of the Care Trust and within team operational policies.
  • Ensure implementation of all Trust Policies and Procedures as required.

(b) Clinical Care/Service Delivery

  • To receive and accept referrals for the service and initiate, lead and coordinate their assessment, making referrals to other professionals as required
  • To manage a personal caseload of clients
