We are looking to recruit a registered nurse to work within the Rapid Response team in North East Derbyshire and Chesterfield.

To manage referrals into our service and provide evidence-based care to support patients in their own home.
This includes working alongside general practice/therapy/district nursing colleagues and services within the acute hospitals to prevent hospital admission and support discharge home.
To assess, plan, implement and evaluate individualised patient care, promoting self care wherever possible in an efficient and clinically effective way, whilst maintaining privacy and dignity.
To provide advice and support to junior staff in the delivery of high quality care and actively support the senior staff in the management of the health care environment.
Our purpose in Derbyshire is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.
We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.
