Description:
Are you a Band 5 Nurse who would like to join our dynamic team? We are seeking an ambitious, motivated, compassionate and experienced Band 5 nurse to support the changing face of community health care. As part of the Urgent Community Response and Virtual Ward initiative, your
role will be essential in providing efficient, integrated, rapid care to our patients with care and respect.
You will be working as part of the Primary Care at Home team predominantly on Unplanned Care and be able to assess, plan, implement and evaluate patient care, allocate work and complete onward referral when required. There is an expectation that you would will have the ability to be flexible to cross cover into planned care if required.
You will be working closely with other members of the MDT and partners, alongside senior clinical staff within your team to formulate a management plan that will support the patient to remain in the community.
Experience in the field of adult community services is desirable but a willingness to learn and contribute is more important. Consideration will also be given to staff who are newly qualified and are passionate about a career in community nursing. A flexible approach is required to accommodate the changing needs of the service along with the drive to influence service development.

You will have regular supervision, mentorship and support from within the Primary Care at Home Team and the Trust.
You must have a driving license and have access to a vehicle in order to carry out your duties. You will be required to work a shift pattern covering 7 days a week from 08:00-20:00.
