JOB OVERVIEW
Surrey Downs Health & Care are looking for a Band 7 Physiotherapist o be part of the SDHC Home First Epsom UCR team. Community experience, experience in ED & rapid responses are required for this post and it is a short term bank position over Winter however you will be onboarded to Bank so will be able to pick up further shifts following this line of work. Home visits for new rapids & follow ups are key duties.
Hours are 34 hours per week in a working pattern of x3 12 hour shifts of 8am-8pm with a 40 minute break.

To undertake specialist physiotherapy and holistic assessment of patients as an autonomous practitioner, interpret results appropriately, use clinical reasoning skills and knowledge of evidence based practice, in relation to the individual’s clinical and psycho-social issues to determine treatment options, recommending the best course of action.
- To undertake treatment planning and implementation of agreed goal orientated intervention with client/carers.
- To contribute towards single assessment and trusted assessment processes
- To provide comprehensive intervention to patients making use of assessment and treatment facilities where appropriate.
- To be responsible for a clinical caseload working within multi-disciplinary teams, sharing care appropriately ensuring patients and carers receive optimum level of therapy and holistic care within available resources.
- To carry out multi-disciplinary screening assessments and discuss outcomes with multidisciplinary team members
