Description:
To contribute in the delivery of high standards of health care to patients within their own homes, care homes, the travelling community and in clinics, by providing proactive, holistic case management and care.
Swaffham and Downham Therapy team are currently looking for a band 6 Temporary Worker physiotherapist.
If you are currently a Band 6 Physiotherapist wanting temporary work in a supportive Community Therapy team, this may be the job for you.
If you are passionate about delivering rehabilitation to patients in the community and working in a close knit therapy team with good multidisciplinary links, we would like to hear from you.
It is essential the post holder has the ability to prioritise their own caseload and has access to transport, to travel to patients across Swaffham and Downham plus the surrounding areas.

We are looking for an enthusiastic and motivated Physiotherapist to join our community rehabilitation team in Swaffham and Downham. You will be part of a friendly and experienced team of Physiotherapists, Occupational Therapists and Community Assistant Practitioners.
There will be an opportunity to work with patients located in community inpatient units and residential homes, as well as providing rehabilitation to patients within their own homes.
You will have the opportunity to progress your clinical knowledge and skills through our in-service training program and will participate in regular supervision. There will also be opportunities to develop your managerial skills through daily operational supervision of junior staff.
Key Areas of Responsibility
To assess and prescribe care packages for patients with long term conditions, the frail & elderly, palliative care and rehabilitation needs for example, to achieve quality of life and independence where possible.
To work within the integrated team to facilitate early discharge from hospital.
To work within the integrated team to prevent unnecessary admission to hospital.
To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.
