Description:
An exciting opportunity has arisen for a Band 5 Community Psychiatric Nurse to join our team in Louth Older Person and Frailty CMHT based at Windsor House. If you are a Registered Health Care Profession, or due to qualify soon, and wish to work in this role we look forward to hearing from you.
Louth OPF CMHT supports older Adults with complex Mental Health problems over the age of 65, and provides specialist memory assessment and treatment for adults of all ages (a new separate specialist memory assessment service is under development).
The role of the Band 5 CPN is to support the team in the assessment and treatment of the patients under our care, and a caseload will be carried by the successful applicant. This is an excellent opportunity for the successful individual to develop their clinical skills and where possible undertake the Band 6 development programme within the team in order to enable career progression which will be explored through the LPFT clinical and managerial supervision process. If successful, you will be supported in professional development.

Adhere to and maintain a high standard of professional conduct at all times as defined by the NMC, NICE, NIMHE and National Service Frameworks for Older People and for Mental Health.
Undertake and record high standards of core assessments in order to support the care and treatment delivered to our patients.
Undertake high standards of clinical care to support the patient, carers / families and the wider team.
Develop and promote skills to enhance the care we deliver to our patient group.
Work flexibly within the service.
Undertake the Band 6 development programme where appropriate.
Ensure all clinical pathways are followed in accordance with LPFT policy.
Manage a defined caseload under the supervision of an experienced member of the team, acting as care coordinator.
Communicate effectively at all times with patients, carers / families and colleagues.
Adhere at all times to the standards and duties of the Band 5 job description as attached.
