THE ROLE:

Join a dynamic and supportive team and embark on a fulfilling journey as a Community Reablement Assistant. In this flexible and rewarding role, you’ll play a vital part in helping individuals regain their confidence and independence after a hospital stay. As the first point of contact for those returning home, you’ll provide essential support and bring positivity to their recovery journey, making a meaningful difference in their lives every day.

YOUR MAIN RESPONSIBILITIES:

• Supporting people with their personal care and empowering them to do as much as possible on their own.
• Monitoring and providing feedback on the progress of people’s goals.
• Assisting with medication and encouraging independence.
• Completing care records and other relevant paperwork.
• Facilitating access to community groups and services.
  No prior care experience? No problem! We’ll provide all the training and support you need to succeed and grow in your role. A valid licence to drive in the UK is required as the role involves travelling locally and lone working. Various hours are available.
