Description:
We are recruiting a Community Service Manager who will be responsible for the management and development of the 4 older people’s community mental health teams (CMHT’s) and the younger person with dementia (YPD) services.
This is a new and exciting role, with the ideal opportunity for an individual who wants to progress in their leadership and management journey and take a leading role in service transformation. The post-holder will have service management responsibility for the 4 teams across the county in Corby, Northampton, Daventry and Rushden. The YPD services are a countywide service.
As the Service Manager, the post holder will promote a culture based on principles of recovery, choice, and person-centred care, in which service users and carers can expect to be active in decision making about their care and treatment. This culture will also be based around intrinsic values of kindness, compassion, and a desire to help others.
Reporting to the Head Services (Older Peoples Mental Health (OPMH) & YPD), the post holder will be responsible for the line management of Operational Managers within their service supporting them to lead and develop their team.
To take overall management responsibility for all aspects of service delivery and operational activity for the service
To be responsible for operational performance indicators including contractual, financial, quality and safety, and people
To provide leadership holding responsibility for operational delivery of service outcomes.
To lead and participate in transformation work and quality improvement
To improve and develop the agreed service model, developing clinical structures, recruiting staff and operationalizing the service
To put in place and monitor arrangements for partnership working among different teams and agencies
To lead the development of integrated pathways for service users accessing the service and other community interventions
Management of buildings where portfolio services are based
To develop robust interagency relationships with key agencies supporting the development of integrated care pathways.
To develop, implement and put in place monitoring arrangements for integrated and partnership working
To ensure Service Users and Carers are included in all the work the service does
To lead on the promotion of equality, diversity and inclusion both in the management of the service and access to the service
Establishing/maintaining an effective Operational Management Team with those direct reports that ensures that the operational teams and the wider service are always supported
To be responsible for the delivery of action plans
Within Northamptonshire we are continuing to transform our core community and crisis pathway services to meet the needs of our local population and meet the ambitions of the NHS Long Term Plan. Through co-production, we are reviewing and developing multi-agency and integrated pathways that are responsive to people’s needs; we are developing place-based teams to enable people to receive care closer to home. Whilst we are redesigning pathways, we are developing an ethos and culture that transforms how we work together as one team sharing care so that service users and carers experience high quality care which is needs led and seamless.
NHFT is an integrated primary care and mental health Trust, providing physical, mental health and specialty services in both hospital settings and out in the community. Because we put the person at the centre of all we do, we focus on delivering care that is as easy to access as possible. This means many of our services can be provided at home, work or in schools. We also provide health services to various prisons and detention centres in Bedfordshire and Cambridgeshire.
