REQUIREMENTS

Licensure / Certification / Registration

• BLS Provider preferred. American Heart Association or American Red Cross accepted.

Education:

• High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
• Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable.

Support health plan members by meeting non-clinical care management needs.

• Provide members with wellness coaching in person, virtually and by phone
• Assist members with applications for public aid, payer specific programs, accessing medication, disability benefits, and other non-clinical issues as needed.
• Locate and facilitate access to community resources for members.
• Provide documentation support related to care management
