SOCIAL INCLUSION INNOVATION COORDINATOR
The Social Inclusion Innovation Lead aims to support the promotion and development of innovative services for participants, to ensure they remain independent and connected to their own community.

ABOUT THE ROLE
The Social Inclusion Innovation Coordinator aims to support the promotion and development of innovative group programs for participants, to remain independent and connected to their own community through a range of physical and cognitive activities and via social participation.
This position is part of the Social Support Program team based at three sites across Kyabram, Stanhope and Tongala communities. The Social Inclusion Innovation Lead is located within the Community Services Department.
This role will supervise a small team of workers, and volunteers and will assist in ensuring current and newly developed programs run efficiently and effectively in accordance with Community Home Support Program (CHSP), Home and Community Care (HACC) and National Disability Insurance Scheme (NDIS) Guidelines and KDHS Policies and Procedures.
