Description:

COMMUNITY SHORT BREAKS PRACTITIONER

Salary: £25,500 per annum (£14,472 pro-rata)
Location: Rhydyfelin, Rhondda Cynon Taf
Contract/Hours: Permanent, Part Time - 21 hours per week – Monday to Sunday between core hours of 09:00 and 22:00 (3 hours per session)

Responsibilities:

A BIT ABOUT THE ROLE

As a Community Short Breaks Practitioner your role will involve managing a small team of Community Short Breaks Workers, providing critical leadership to ensure an effective, efficient service covering short breaks, groups and other support agencies in Rhondda.
You’ll oversee the organising and supervising of activities with an aim to promote socialization, independence and personal development.
You’ll collaborate with families and other professionals to tailor activities and support to the specific needs of each child or young person.

The aim of this role is to co-ordinate and direct the support of children, young people and families, by agreeing interventions, which enable positive and agreed outcomes to be achieved. You will help do this by:

  • Leading a team to support children and young people with disabilities to reach their potential
  • Building positive relationships with families, professional colleagues and within multi-agency networks
  • Leading on team meetings and conducting 1 to 1 supervision
  • Running a range of groups and activities to help children and young people grow
  • Supporting those in our care to access a range of stimulating experiencesTaking responsibility for day-to-day resource and rota planning and caseload management to ensure high quality services are delivere
