Description:

The Clinical Nurse Advisor will provide clinical support, assistance and professional expertise to all callers to the Community Nursing Triage, including both healthcare professionals and patients within Bracknell. The service is the main point of access for Bracknell Community Nursing taking, referrals from professionals and patients requiring healthcare services and the Trust Health Hub.
The Clinical Nurse Advisor, will offer clinical triage and support where appropriate. This may range from signposting the referral co-ordinator/caller to the appropriate service, making a direct referral or offering clinical advice.
The post holder is expected to represent the organisation in a professional and courteous manner at all times and provide advice and information using a level of clinical understanding and professional judgement.

To work as a key member of the Triage Team.
To offer sound clinical advice that oversees entry into our service.
To maintain up-to-date Statutory and Mandatory training and be prepared to rotate into clinical services as necessary to maintain clinical skills.
To hold professional registration and adhere to professional codes of practice and conduct.
To work in conjunction with administrative support staff to provide high quality, clinical triage using knowledge, skills, critical thinking and professional judgement.
To assist the administrative support staff where required by accepting the transfer of cases requiring additional clinical advice, reviewing recorded clinical information and offering advice.
To control and manage calls effectively by using skilled questioning, active listening and excellent verbal communication skills in a calm, professional manner.
The Clinical Nurse Advisor will possess the ability to prioritise tasks, cases and situations appropriately to ensure effective delivery of services during periods of high demand.
To liaise and communicate clinical information verbally and/or via telephone to Clinical Nursing Staff as appropriate to ensure continuity of care for the caller.

Full Driving Licence and vehicle for business use.
A Team player with good nursing care knowledge/skills and excellent communication skills.
As a Community Triage Nurse, no day is the same and you will be communicating with patients of varied needs, from wound care to end of life care. Please note you must currently be in the UK with access to a car for work purposes to be eligible for this role. This role will suit a qualified nurse coming to the end of their career or have injury where they can no longer provide nursing care but would be able to utilise your nursing care knowledge when triaging calls.
