We are recruiting a Band 5 Staff Nurse to our Palliative Care Support Team. We support patients who are end of life across Bradford in the community to provide high quality care to patients at the end of life. Working closely with community nursing teams, Fast Track team and Community Palliative Nurse Specialist, you will support patients to prevent unnecessary admissions to hospital, and support them in their preferred
place of care.
In addition, you will work alongside other key members of the primary health care team, other health and social care professionals, and carers, to
develop and implement personalised care plans for patients at the end of life.

To deliver high quality time limited programmes of care for patients who are at the End of Life (EoL). To support the development of the palliative
