Description:

Community Staff Nurse - Health Visiting - 37.5 hours
This Registered Nurse role will support Health Visitors to the delivery of the Healthy Child Programme (HCP) 0-5 years. The post holder will have delegated responsibility for the assessment of health needs, implementation and evaluation of care as part of the Healthy Child Programme, at Community, Universal, Universal Partnership and Universal Partnership Plus level. This includes children and families with complex health, social and emotional needs, children presenting as a cause for concern, subject to safeguarding and cared for children.
The post holder will be based within a locality (neighbourhood) and will work with the health visiting teams in that locality. Tameside Health Visiting services are delivered within the Tameside Metropolitan Borough boundary.
The role involves working in partnership with children, young people, families, primary care, schools, statutory & voluntary services and other partner agencies, in order to facilitate health-enhancing activities and improve health outcomes.
This is a fixed-term developmental role, providing experience and insight into the role of Specialist Community Public Health Nurses (SCPHNs), and with the expectation that the post holder will be well placed to apply for training in SCPHN Health Visiting.
Within the corporate caseload and local community, and as delegated by the Health Visitor, the post holder will:
Contribute to the safe and effective delivery of the Health Visiting services through participating in the functioning of the corporate team and caseload.
Contribute to the delivery of care for children, young people and families in the community which is accessible, appropriate, culturally sensitive, safe, person-centred, evidence-based and effective.
Deliver services to children and families within homes, children’s centres, preschool day care settings, schools, primary care and wider community settings, taking into account the lifestyle, ethnicity, gender, cultural and other factors which impact on physical, mental or emotional health
Work in partnership with service users, other health visiting and school nursing team members, care providers and other agencies. Develop, participate in and maintain interagency networks.
Provide advice and support, signpost to other services and promote selfcare, as appropriate, to children, young people and families regarding their care.
