Description:
CLINICAL RESPONSIBILITIES Identify and carry out routine and specific clinical assessments for a designated caseload or as delegated by a senior member of staff. Report assessment findings through the most appropriate media within established protocols/ guidelines. To demonstrate ability to independently plan, implement and progress rehabilitation programmes in liaison with qualified staff including therapeutic handling, patient education, provision of exercise and outdoor mobility practise activities of daily living, communication therapy and other alternative options. Make judgements as to the patients response to these activities, taking action including modifications to the programme, as required and report as appropriate giving feedback to senior staff as necessary in line with established protocols/ guidelines according to competencies.
To prepare, organise and instruct patients in routine and specific activities as identified within a given caseload, modify as necessary in line with established protocols and competencies providing feedback to qualified staff and colleagues. This may include Health Promotion and Education. To assess for, provide and instruct patients as to the use of designated specific equipment making specific modifications as required in line with established protocols. Carry out routine provision and modification of equipment as required by the service.
Make appropriate observations of the patient during activities and report abnormalities to qualified staff. To liaise with external agencies with regards to follow up and discharge planning providing feedback to qualified staff as appropriate. To assist in the provision of education and exercise sessions as needed. To monitor and maintain specific equipment in terms of fitness for use and stock levels.
To take responsibility with regards to specialist equipment and feedback as appropriate. To participate and contribute to multidisciplinary and team meetings To be responsible for maintaining accurate and comprehensive patient treatment records in line with organisational record keeping standards, including preparation of reports to consultants and GPs. To maintain patient records accurately and legibly in accordance with trust guidelines. To demonstrate skills in motivating clients and/or carers to engage in the therapeutic process.
To have regard to moving and lifting relevant equipment between locations e.g., clinical equipment. To be flexible to the demands of the environment. To communicate effectively with patients with stroke who may have barriers to understanding. To communicate effectively with patients/carers/colleagues/outside agencies on a range of matters with tact and diplomacy where situations/ matters may be sensitive and/ or difficult.
To communicate condition related information with patients/carers/MDT (verbal or written) working collaboratively with health and social care and external agencies to ensure maximum rehabilitation potential is achieved. To support Senior staff in dealing with complex situations and matters appropriately. Participate in the health and social care review assessments for patients in conjunction with the stroke team. Promote equality, diversity and rights and contributes to the protection of individuals from abuse.
Work as a member of a team, developing and promoting effective communication and relationships with all contacts during work activities. Maintain the health and safety of self and others, including basic first aid in an emergency situation. Any other clinical responsibilities delegated. MANAGEMENT RESPONSIBILITY To contribute to interagency/multi-disciplinary team building.
To be aware of, adhere to and implement service and team plans and policies. To deal with initial complaints sensitively bringing it to the attention of the line manager, following service policies. To assist the Community Stroke Team in providing an Early Supported Discharge service and achieving the key performance indicators. Provide effective, appropriate, and timely rehabilitation in liaison with qualified staff.
Any additional management duties as delegated EDUCATION AND TRAINING To deliver knowledge and skills to staff as appropriate. To plan, undertake and review specific liaison/key trainer/link person duties as delegated. To participate in the induction of new staff / students. To assist with the identification of training needs within the team.
Plan and undertake delegated education, health promotion, public relations, and marketing activities. To develop the ability to reflect on and evaluate training provided. To contribute to the teams training programme. To attend training /courses as directed by an individual personal development plan and the needs of the service, and to disseminate this information to colleagues to support best practice.
5. PROFESSIONAL RESPONSIBILITIES To act in a professional manner whilst undertaking duties To collect, collate, record and report in appropriate form and content clinical and non- clinical activities to the most appropriate person; taking action as required. To maintain clear, accurate, documentation (using electronic systems) according to service standards and support others in maintaining standards. Adhere to code of conduct for Assistants as identified through the CSP/ RCOT /RCSLT and other relevant professional bodies.
Work within relevant local and national guidelines (e.g. NSF and NICE Guidelines) Act in ways that maintain and support equality and value diversity within all clinical and non -clinical tasks, activities, and interactions. Identifying and responding appropriately to individual differences. Maintain a positive role model for new staff and students.
