Description:

The Newbury Older Peoples Mental Health Service is looking for an enthusiastic, caring and highly motivated community support worker to join our friendly and supportive team.
We are a multi-disciplinary team with specialist skills in caring for older people with mental health needs and people with dementia. Therefore, we are looking for someone who has a passion for working with this client group.
In this role y ou will be support ing the co-ordinat ion and facilitation of evidence-based psychosocial group work for people with a diagnosis of dementia (Cognitive Stimulation Therapy), working alongside colleagues from psychology and the memory clinic service .
You will also be working alongside colleagues in the Community Mental Health Team offering mental health monitoring and support a s well as supporting and guiding patients to access and engage with community resources.
Y ou will receive regular individual supervision, and there will be opportunities for continuing professional development (CPD ) .
Co-facilitate C ognitive S timulation T herapy groups.
To provide support to M ental Health P rofessionals in the provision of psychosocial assessment and intervention for behaviours that challenge in dementia.
Support the wider service to complet e physical health monitoring.
Be responsible for a caseload of patients working collaboratively with mental health professionals . Offering assessment/treatment and monitoring in line with the patients care plan.
Provide social and emotional support to patients and carers within the context of trusting and therapeutic relationships.
Enable the patient to maintain and develop their social skills, encourage social contacts and promote social networks if appropriate .
Liaise with other agencies in both statutory and voluntary sectors, eg Gp’s , Local Authority, Alzheimer’s Society, Age Concern, etc.
Maintain clinical records as per trust policy and clinical need.
Participate in the process of clinical supervision and appraisal.
Attend study days and training courses as agreed and consistent with the needs of the service.
Attend team meetings as appropriate .
