Description:

An exciting opportunity has arisen to join our triage team as a Community Triage Clinician in the North Locality Hub. You will be responsible for triaging incoming referrals assessing the patients’ needs, determining the priority of care and processing the referrals accordingly. This will involve contacting the patient and other professionals via telephone, to provide an initial assessment and advice. The position will be approximately 80% triage with the remaining time providing the opportunity to work face to face with patients, providing nursing care.
We offer an excellent training and support package to enable you to both maintain your current clinical competencies as well as developing new skills and in a community setting. There is no need for applicants to have previous triage experience as all training and support will be provided.

To process referrals by evaluating the information received and allocating to the appropriate service indicating the priority response necessary.
To document triage notes in order that the patients’ record is complete and for audit purposes.
To use information technology to process the referrals for the 80% triaging role.
To liaise and communicate with the general public, other health care professionals, and multi-agencies on a daily basis.
To assess, prescribe and deliver care packages for patients, for example with long term conditions, palliative care and rehabilitation needs, to achieve quality of life and independence where possible.
To work within the integrated team to facilitate early discharge from hospital.
To work within the integrated team to prevent unnecessary admission to hospital.
To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.
