Description:

Complex Case Manager Substantive post 37.5 hrs per week within the ICB
Project a positive image of the CHC Team both within and outside of the service.
Responsible for undertaking CHC assessments and the management of Complex patients with identified healthcare needs
Responsible for the line management of B6 Clinical Nurse Assessors
Responsible for ensuring KPI’s and QIPP savings
Quality Assurance
Oversight of Complex LD Cases
Liaising and active engagement with CHC stakeholders
Facilitating Level 1 dispute Meeting
To participate with the allocated duty role within the CHC Team
Ensuring ICB Values are achieved

Complex Case Manager

  • To assess, case manage, monitor, and formally review the health needs of those patients funded by CWICB within Coventry, Rugby, and Warwickshire North places as well as those cases who are placed out of county placements receiving care under CHC.
  • To review existing historical joint funding care packages for learning disabilities clients where the issues are considered to be standard rather than complex
  • To carry out verification and Quality Assurance of all DSTs and Review Assessments as one of the primary assignments for the role.
  • To coordinate and provide all needed support to the Legal Team in all Legal Cases and report to the line Manager of the progress of all cases.
  • To liaise with the ICB DOLs Team in order to provide the team with any required clinical information as may be required and provide update to the Clinical Commissioning Manager of the progress.
  • To co-ordinate and ensure timely and effective dispute process. To Lead in Level 1 dispute and if not resolved, this will be escalated to Level 2 stage.
