Complex Case Practitioner

Be responsible for the holistic and clinical assessment, planning and implementation, evaluation and facilitation of care to individuals, groups of patients and their families on the caseload encompassing a proactive, rehabilitative and an active self-care approach. To act as an autonomous practitioner taking on going responsibility for the complex care coordination of appropriate patients on their caseload. Contribute to the organisation of multi-disciplinary, patient centered care. Assess and develop an individualised plan of care to enable proactive management of patients with complex needs, which may be identified by a risk stratification process.
Develop and support reduction in hospital admission targets within Integrated Care Strategy. Take an active role in health education of the individual and community by promoting self-care for long and short-term conditions, utilising motivational interviewing techniques and provide both planned and opportunistic care based upon evidenced based information
