The Complex Discharge team at the University Hospital Southampton NHS Foundation Trust (UHS) are looking to appoint to an opening for a Band 7 Team leader role, initially on a fixed term/ secondment opportunity. The role is fast-paced, varied, and incredibly interesting, supporting the wider hospital and enabling patients to progress closer to their discharge following acute care. We are looking for someone with good communication skills, who is reliable and driven, with a clear passion for excellent patient care. The post holder will have a variety of leadership responsibilities within the immediate team and also hold a diverse patient caseload according to the service needs. We are a really friendly team and like to support one another and have fun in our work, helping patients to remain independent and have a safe, quality discharge on leaving our hospital.

The post holder is responsible for managing the teams undertaking the assessment and discharge planning of patients within the defined clinical area, working under the direction of their Complex Discharge Operational Lead. In addition, act as a case manager using their clinical knowledge and experience to support the day-to-day operational functions of the teams, completing their own assessments and interventions where indicated
