Description:
The post holder will be the Consultant Psychiatrist to the Greenwich West Anxiety, Depression, Personality Disorders and Trauma Team (ADAPT) with Oxleas NHS Founda on Trust. This post has
been covered temporarily for the past 3 years by the Associate Specialist in the team ‘ac ng up’.
The post holder will carry no responsibility for inpatients but will be part of the bed management strategy within the Community Mental Health (CMH) Directorate. The Directorate takes a whole system approach to bed management with twice daily meetings between community and inpatient clinicians to ensure that beds are used in the most efficient manner. There is additional community support in the form of our admission avoidance team, the home treatment team and our crisis house
before patients may need a hospital bed.
- The ADAPT Pathway provides focused, therapeutic interventions to working age adults residing within
the Greenwich Borough who require care and treatment for anxiety, depression, affective disorders
(without psychosis), personality disorders and trauma.
The service aims to be flexible and accessible. The pathways of care aim to offer individuals, choice
and involvement regarding their treatment, actively encouraging their participate on in the planning
and implementation of their care. A personalised care plan will be devised in consultation with the
service user and their Carer/family where appropriate.
The Greenwich ADAPT Team was formed as part of a reconfiguration of Community Mental Health
Services that occurred in September 2015. The reconfiguration of Community Services was part of
the directorate’s plan to ensure efficient delivery of mental health services whilst delivering high
quality care.
- The post holder will be responsible for the care and treatment of patients within the team.
This will involve attending multi-disciplinary team meetings to discuss new referrals to the
team. The post holder will be expected to attend zoning meetings during the week to help
the team prioritize contacts with patients, especially those service users who are
deteriorating and presenting with increasing risk.
- The post holder will be expected to chair review/CPA meetings, offer crisis appointments to
patients who are in the red zone and offer home visits and telephone advice to general
practitioners and other professional who may be involved with the care of service users on
the ADAPT caseload.
- The post holder will have timetabled outpatient clinics each week. On average in the
Medical Clinics there are 4-6 patients and in the Assessment Clinics there are 2 patients in
each clinic. It is anticipated there will be 4-5 clinics, constituting direct clinical care each
week.
- There is protected time for administrative work, and the post holder will have access to
appropriate digital/technological support to ensure timely communication with primary
care colleagues.
- There may be some Mental health act related work including attending tribunals for
patients on community treatment orders (CTO), but this is generally unlikely within this
part of the community pathway.
- The post holder will have supervision duties for the Trust Grade/ Trainees/ Specialty Doctor
working within the team. They will be expected to provide weekly supervision and
discussion of the caseload and any relevant clinical issues.
- The consultant will in collaboration with the team manager and senior colleagues review
the caseload to ensure that there is throughput in the team.

improvement activities, and will be supported to use their own SPA time for any personal
research, teaching or Qi
- There will from time to time be ‘directorate’ activities that the consultant will be requested
to support/lead on which may include complaint or serious incident investigation panels.
- The CMH directorate aims to provide holistic care, with a focus on wellbeing and physical
health, for patients that we know are likely to die younger than their peers. The consultant
will be expected to lead on physical health care improvement within the team.
- The consultant will be invited to be part of the monthly Greenwich medical peer group. The
bi-monthly directorate medical meetings and the quarterly Trust MAC.

The service aims to be flexible and accessible. The pathways of care aim to offer individuals, choice and involvement regarding their treatment, actively encouraging their participation in the planning and implementation of their care.
A personalised care plan will be devised in consultation with the service user and their Carer/family where appropriate.
Whilst Pathways of Care utilises a team approach to the delivery of care, every individual receiving care will be allocated either a health care professional (HCP) or care coordinator. This worker will be responsible for ensuring that care is delivered in accordance with the care/treatment plan, and that the needs of the individual are regularly reviewed, risks and care plans modified accordingly
