Responsibilities:

  • Fostering healthy team processes that develop a person-centred continuum of care, applying the LEADS framework in a health care environment
  • Facilitating collaboration between respective program areas and their networks across the continuum of care while linking with patients, families and community organizations
  • Assisting in program planning and evaluation, ensuring practice is evidence-informed
  • Participating in the orientation and mentorship of new team members
  • Supporting a quality approach to service delivery
  • Assisting with the development of the consistent application of policies related to continuing care programs and services
  • Facilitate a culture of learning and excellence for staff and student
