Description:

The post-holder will assist the Continuing Health Care Co-ordinator in coordinating and sustaining a continuing health care model and service to meet the needs of the adult population of Merthyr, Cynon, Rhondda and Taff Ely.
The post-holder will be responsible for ensuring that the service provided to individuals within the community setting meeting the Eligibility Criteria for Continuing Health Care is of the highest clinical standard, and also be responsible for coordinating, monitoring and evaluating service provision in conjunction with District nursing team Leaders, ensuring effective use of resources.
The post-holder will have extensive knowledge of this client group and the experience to provide advice on elements of Continuing Health Care and NHS funded care.
The post-holder will have a direct relationship with discharge liaison to facilitate complex discharges.
The post-holder will work with the Continuing Health Care Co-ordinator to deliver ongoing educational programmes to ensure district nurses have the skills to make robust Continuing Health Care assessments, work in partnership and implement the Continuing Health Care policy.

  • To facilitate, in conjunction with the District Nurse service the collation of the multi-disciplinary assessments for complex individual patients and application of the continuing health care eligibility criteria.
  • Maintain a database of all Continuing health care and joint funded clients.
  • Quality assures all nursing assessments and Continuing healthcare documentation to ensure correct process is being following using guidance from the CHC framework.
  • Liaise with the Marie curie service on a regular basis to monitor its use and quality of service.
  • Complete relevant CHC documentation for approval panel and present cases to senior nurses.
  • Source and commission care packages for CHC Fast track patients in the community in conjunction with the DN service.
  • Monitor the quality of service via clinical audit, patient satisfaction, reporting to the Continuing Health Care Co-ordinator.
  • Report any concerns related to patient care following reviews or assessments to the CHC Coordinator. Work in partnership with Social services, CIW, adult protection as required.
  • Using knowledge acquired through experience and training, provide support and guidance to the District Nurse Team Leaders in their case management responsibilities for packages of care.
  • Maintain up to date records of statutory/mandatory training attendance & compliance for self on ESR.
  • Liaise with the appropriate clinical specialists e.g. Doctors, Allied Health Professionals, Clinical Nurse Specialists, in accordance with patients needs.
  • To support the DN service in the case management of specific continuing health care packages.
  • To attend case conferences as necessary for complex cases supporting the DN service advising on the application of the eligibility criteria for continuing health care.
  • Communicate sensitive information concerning patients medical conditions, which requires persuasive and reassurance skills. Some Continuing Healthcare patients have special needs/learning disabilities.
  • Informs the Continuing Health Care Co-ordinator on the identified clinical risks associated with continuing health care packages, advising on the skills and qualifications required by both registered and non registered staff to minimise the risk.
  • Responsible for maintaining own continuous professional development and NMC registration.
  • To contribute to identifying stakeholder requirements including consumer satisfaction programmes/ surveys.
  • To frequently participate in multi-disciplinary/multi-agency forums to enhance closer working relationships, including providing professional advice and support.
  • The postholder will work in partnership with other agencies including local authorities and the voluntary sector.
  • Responds to requests to consult on a wide range of documents identifying the potential impact on local service provision reporting to the Continuing Health Care Co-ordinator.
  • In conjunction with the Continuing Health Care Co-ordinator communicate with patients/carers and their representatives, ensuring all documentation is available for their perusal and face-to-face meetings to discuss Continuing Health Care applications and appeals.
  • Monitor standards of care, including the physical, social, psychological & spiritual needs of clients (& significant others) in collaboration with the District Nursing Team Leaders.
  • Support the DN Team Leader’s in ensuring all clients have an individual & comprehensive plan of care, which promotes & maintains wellbeing & prevention of illness.
  • Participate in reviewing client satisfaction with the service, contributing to the development of action plans.
  • Actively participate in the development and review of policies and procedures within Continuing Care.
  • Contribute to the interpretation and implementation of national standards & frameworks locally relevant Continuing Care.
  • Participate in complaint investigation/resolution/ multi agency disputes and appeals.
  • To attend Safeguarding meetings as directed.
  • Assist in training staff on the implementation of policies and guidance relating to NHS funded nursing care and NHS continuing healthcare. Ensure relevant staff within District nursing services has appropriate support and training in relation to implementing continuing healthcare and funded nursing care. Assist where appropriate with the completion of the Decision support tool.
  • To travel, when the service requires, to other sites across CTMUHB.
