REQUIREMENTS

Licensure / Certification / Registration:

One or more of the following preferred:

• Licensed Social Worker credentialed from the Indiana Behavioral Health and Human Services Licensing Board.
• Licensed Social Worker specializing in Clinical Practice credentialed from the Indiana Behavioral Health and Human Services Licensing Board.

Education:

• High School diploma equivalency with 2 years of cumulative experience OR Associate’sdegree/Technical degree OR 4 years of applicable cumulative job specific experience required.

Provide coordination of patient care throughout the healthcare continuum.

Responsibilities:

• Provide coordination of communications with patients, families, physicians, and healthcare team members.
• Coordinate patient scheduling with appropriate resources or clinical services and maintains required documentation.
• Support standard patient access initiatives across the healthcare continuum.
• Provide support with navigating the healthcare system and assists patients with utilizing available community support services.
