POSITION SUMMARY:
The Coordinated Entry Intake Specialist acts as an entryway to the Coordinated Entry process. They are a critical member of the Coordinated Entry Team and work closely with 211. The Intake Specialist provides telephonic assessment and connection to resources to those who are experiencing homelessness in Milwaukee County. The goal of this work is to prevent literal homelessness whenever possible, and to provide resources to the next best step within the Coordinated Entry system as a result of the assessment. The Coordinated Entry Intake Specialist will provide quality professional services to internal and external customers and implement IMPACT’s philosophy of customer service, community focus, collaboration, expertise, compassion and integrity.

QUALIFICATIONS & WORK SCHEDULE:
- Bachelor’s degree in Social Work or other human service studies preferred, but strong background of applicable work experience may substitute.
- Experience working with people experiencing homelessness strongly preferred.
- Demonstrate proficiency with MS Office products, including Word, Excel, Outlook, and PowerPoint, as well as HMIS
- Reside in the State of Wisconsin and within 50 miles of IMPACT.
- Access to working vehicle, car insurance, and license to be able to respond in the community via driving.
The position generally works Monday - Friday from 9a-5p. The position is hybrid, onsite hours may be located at the IMPACT office or at a CE Assessment Site. However, for training purposes, the candidate will be required to work onsite for the first 60-90 days of employment. The position will work, as necessary to achieve success, to coordinate work, consult with colleagues, attend meetings, and participate in agency activities and events.

Responsibilities:
1.Provide assessment of current housing circumstances and needs, problem solving, information and referral, and crisis intervention when necessary in an empathetic and nonjudgmental manner.
2.Complete a standardized assessment for connection to the Coordinated Entry system.
3.Assist customers in developing a triage plan within the scope of eligible family, health, and social services that appropriately meets their needs.
4.Accurately document customer contacts through client profiles and case notes in a computerized database and other means.
5.Provide advocacy for customers experiencing difficulties or lacking in abilities needed to make effective contacts with agencies, programs or groups.
6.Participate in ongoing IMPACT training and staff meetings.
7.Participate in ongoing Continuum of Care meetings as appropriate.
8.Keep informed of and demonstrate knowledge of all special service programs.
9.Ensure that IMPACT is compliant with applicable federal, state, and local laws ensuring client confidentiality.
10.Assure that data entered into the Wisconsin HMIS database by IMPACT 211 is accurate, up-to-date and entered according to the standards and practices required by the Milwaukee County Continuum of Care and all governing bodies.
11.Cross train with other Coordinated Entry positions in order to support or fill in when necessary for Prevention Navigation and/or Mobile Screening.
12.Participate in trainings related to homelessness, HMIS and any other trainings associated with, shelter systems and housing and others as needed.
13.Actively participate in and support the organization’s DEI initiatives
