QUALIFICATIONS:

Required:

  • At least 2 years of experience in community outreach
  • Proven ability to develop and implement successful outreach programs and establish community partnerships
  • Excellent communication, presentation, and interpersonal skills
  • Ability to work independently and as part of a team
  • Knowledge of community health needs and trends in the service area
  • Fluency in Spanish or other languages prevalent in the service area is preferred

Responsibilities:

  • Program Planning and Implementation: Research community health needs and trends to inform the development of outreach programs, health fairs, and educational events.
  • Collaborate with internal stakeholders, such as physicians and department heads, to secure speakers and resources.
  • Coordinate logistics, including venue selection, marketing, and registration.
  • Community Partnerships: Build and maintain relationships with key community organizations, local health departments, schools, and businesses to advance shared health goals.
  • Identify opportunities for collaboration, joint programming, and resource sharing.
  • Program Evaluation: Collect and analyze data to assess the effectiveness of outreach programs in reaching target populations and promoting healthy behaviors.
  • Prepare reports and recommendations for program improvement.
  • Resource Coordination: Facilitate the provision of resources, including staffing, equipment, and supplies, necessary for program delivery.
  • Liaise with internal departments to secure support services, such as interpretation and transportation.
  • Health Education: Deliver health education presentations and workshops on a variety of topics, such as chronic disease management, preventive care, and wellness strategies.
  • Provide one-on-one health counseling and connect individuals with appropriate medical services.
  • Community Representation: Represent Clara Maass Medical Center at community events, health fairs, and coalition meetings.
  • Promote the medical center’s services and programs to diverse audiences.
  • Cultural Competency: Ensure all outreach activities are culturally sensitive and tailored to the needs of diverse community groups.
  • Stay abreast of community trends, cultural events, and health issues impacting priority populations.
  • Documentation and Reporting: Maintain accurate records of outreach activities, including the number of participants, presentations conducted, and community partnerships established.
  • Prepare regular reports for leadership and funders as required.
