QUALIFICATIONS:
Required:
- At least 2 years of experience in community outreach
- Proven ability to develop and implement successful outreach programs and establish community partnerships
- Excellent communication, presentation, and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of community health needs and trends in the service area
Fluency in Spanish or other languages prevalent in the service area is preferred

Responsibilities:
- Program Planning and Implementation: Research community health needs and trends to inform the development of outreach programs, health fairs, and educational events.
- Collaborate with internal stakeholders, such as physicians and department heads, to secure speakers and resources.
- Coordinate logistics, including venue selection, marketing, and registration.
- Community Partnerships: Build and maintain relationships with key community organizations, local health departments, schools, and businesses to advance shared health goals.
- Identify opportunities for collaboration, joint programming, and resource sharing.
- Program Evaluation: Collect and analyze data to assess the effectiveness of outreach programs in reaching target populations and promoting healthy behaviors.
- Prepare reports and recommendations for program improvement.
- Resource Coordination: Facilitate the provision of resources, including staffing, equipment, and supplies, necessary for program delivery.
- Liaise with internal departments to secure support services, such as interpretation and transportation.
- Health Education: Deliver health education presentations and workshops on a variety of topics, such as chronic disease management, preventive care, and wellness strategies.
- Provide one-on-one health counseling and connect individuals with appropriate medical services.
- Community Representation: Represent Clara Maass Medical Center at community events, health fairs, and coalition meetings.
- Promote the medical center’s services and programs to diverse audiences.
- Cultural Competency: Ensure all outreach activities are culturally sensitive and tailored to the needs of diverse community groups.
- Stay abreast of community trends, cultural events, and health issues impacting priority populations.
- Documentation and Reporting: Maintain accurate records of outreach activities, including the number of participants, presentations conducted, and community partnerships established.
- Prepare regular reports for leadership and funders as required.
