DIABETES PROGRAM COORDINATOR
(Haitian Creole or Spanish Speaking highly preferred)
Job Location: Midtown Manhattan, NY; 4 days in office, 1 day remote, with occasional travel to clinical sites in NY & NJ areas.
Job Schedule: Mon-Fri, Day Shift

Pay Rate: $60,000-$70,000 per annum
JOB DESCRIPTION:
VitalCheck Wellness is seeking a Program Coordinator for our Diabetes and Chronic Disease Management Program, to coordinate the program, make sure there is patient adherence and provide education to employees of our clients in NY and NJ area. We are looking for someone passionate about improving patients lives and delivering high level customer service.
REQUIREMENTS:
- Bachelor’s degree in nursing, health administration, or a related field.
- Active license of Licensed Practical Nurse (LPN) in NY or NJ (preferred).
- Three to five years of experience in nursing, health administration, or a related field..
- Experience working with diabetes patients. Certified Diabetes Educator strongly preferred.
- Proficient with computers, Microsoft Office, EMR systems.
- Customer service-oriented and have great bedside manner.
- Strong communication skills; both written and verbal.
- Ability to work independently with little supervision.
- Bilingual with Haitian Creole or Spanish speaking strongly preferred.
- Must be able to reliably commute throughout NYC and/or NJ.

Responsibilities:
- Coordinate between patient, clinical and operations partners to set up appointments, follow up, and get feedback.
- Make sure high adherence rate to the program in accordance with our guideline.
- Provide education around diabetes management and exercise advice to help clients establish healthy eating habits and lifestyle.
- Support clients in diabetes management and other aspects of well-being and lifestyle.
- Establish and maintain rapport and good relationship with patients.
- Maintain clients documentation.
