The Care Coordinator will consult and collaborate with an interprofessional Virtual Care Team (VCT) on a weekly basis and help participants navigate to appropriate care providers and/or resources as needed. The Care Coordinator will also coordinate and facilitate educational group sessions on an as-needed basis. The incumbent will work collaboratively with an interdisciplinary team of internal and external stakeholders, including healthcare providers, research staff and people with lived experience, to achieve project deliverables, such as conducting environmental scans, developing resources, preparing project updates and writing manuscripts.
