Description:

- Bachelors or advanced degree in social, behavioral sciences, medical or health related area
- Experience in planning, implementing and evaluating a health program
- Experience providing consultation and technical assistance to local agencies
- Experience writing instructional/educational, grant or guidance materials
- Experience in presenting education/training programs or public speaking
- Experience and/or training in public health policy, systems, environmental changes
- Experience and/or training in community engagement
- Experience and/or training in health equity and social determinants of health
- Experience with computer software (e.g., Word, Outlook, PowerPoint)

Unless required by legislation, the selected candidate will begin at Pay Grade 12, Step 1 of the 1199 Pay Range Schedule ($30.37 per hour), with an opportunity for pay increase after six months ($31.91) of satisfactory performance and then a yearly raise thereafter.
EDUCATIONAL TRANSCRIPT REQUIREMENTS:
Official educational transcripts are required for all post-high school educational accomplishments, coursework or degrees claimed on the application. Applicants will be required to submit an official transcript prior to receiving a formal offer of employment. Failure to provide transcripts within five (5) working days of being requested will cause the applicant to be eliminated from further consideration.

Qualifications
- 6 mos. exp. in providing direct service &/or consultation in a public health, community health, hospital &/or clinic setting, to include exp. in program evaluation, grant review, developing & presenting training programs & in facilitating &/or working collaboratively with coalitions or groups;
- Completion of graduate core coursework in public health, health care, health administration or related field; must be able to provide own transportation.
- Or
- 18 mos. exp. in providing direct service &/or consultation in a public health, community health, hospital &/or clinic setting, to include exp. in program evaluation, grant review, developing & presenting training programs & in facilitating &/or working collaboratively with coalitions or groups;
- Completion of undergraduate core coursework in public health, health care, health administration or related field; must be able to provide own transportation.

Responsibilities:
- Acts as program consultant to Creating Healthy Communities grant projects.
- Provides technical assistance to assigned sub-grantee agencies regarding comprehensive, population based projects through site visits and other contacts for compliance with the grant requirements.
- Works with Health Promotion team in providing technical assistance to funded projects in meeting program objectives.
- Plans and conducts workshops, conferences, trainings and technical assistance for funded projects and a variety of public/private health personnel.
- Assesses training needs, identifies relevant partners, and identifies resources and educational materials.
- Provides technical assistance to funded project partners in adopting policies, environmental and system changes in priority communities.
- Assists in non-traditional program evaluation methods.
- Provides technical assistance on the community engagement spectrum and inclusion of groups historically left out of decision making.
- Utilizes software and processes necessary reports when appropriate.
- Approves budget revisions, expenditure reports, project materials, and other reports when submitted by projects.
- Tracks performance measure in Clear Impact using Results Based Accountability (RBA) methods.
- Assists Creating Healthy Communities program manager and other staff in preparing grant proposals, data reports, and other administrative duties.
- Contributes to Preventive Health and Health Services Block Grant (PHHSBG) plan, annual report, and evaluation and other CDC Cooperative Agreements.
- Assists in developing requests for proposals for local health promotion projects.
- Reviews grant/contract proposals submitted to the Ohio Department of Health (ODH) for funding and determines appropriateness for funding, revisions, and special conditions for funding.
- Conducts literature reviews and attends trainings, meetings, and conferences to keep abreast of trends, best practices, other developments, and to expand knowledge related to health promotion.
