JOB OPPORTUNITY - TEAM COORDINATOR, DAY TREATMENT PROGRAM - FULL-TIME PERMANENT
Youthdale Treatment Centres is a leading non-profit and charitable community agency dedicated to serving children, youth, and families from diverse backgrounds and communities across Ontario struggling with complex mental health needs. We work collaboratively through active partnerships between the agency, youth, and families to create meaningful engagement and effective treatment, resulting in positive outcomes.
Reporting to Director of School Program, Team Coordinator (Day Treatment program) will join the Youthdale Section 23 programs, Day Treatment programs located in either Central Toronto Academy or Jesse Ketchum Public School. The Youthdale school programs are a collaborative partnership with the Toronto District School Board. As an integral member of a Day Treatment school team, school staff ensure a safe, positive and therapeutic milieu with the goal of assisting students in the development of both the necessary academic/learning skills and appropriate social/emotion behaviour’s so that reintegration into a community school system can be successfully achieve.

Hours: 37.5 hours per week
Status: Full-Time Permanent
Union/Non: This is a unionized position
Rate of pay: $24.8287 per hour
QUALIFICATIONS
- Must have Child & Youth Worker (CYW) diploma, Developmental Support Worker (DSW) or Social Service Worker (SSW) diploma or degree/diploma in related field.
- Minimum 2 years’ experience in children’s mental health.
- Demonstrate strong clinical skills and a good understanding of learning styles and learning disabilities.
- A strong understanding of the philosophy, structure and routines of the program and an ability to impart and teach these skills to team members, Board teachers and host school partners.
- Strong interpersonal and communication skills with proven ability to establish partnerships.
- Sound knowledge and/or experience in the mental health field and an ability to manage the emotional and behavioral needs of clients.
- Developed skills in problem solving, critical thinking and organization.
- An ability to work on a multi-disciplinary team; understanding of group dynamics, role-play, behavior management techniques and the nature of Day Treatment
- An ability to support a youth in an educational environment.
- A strong understanding of learning disabilities
- Effective counselling skills
- Valid Safe Management Group certification and CPR/First Aid.
Responsibilities:
ACCOUNTABILITY
This position reports directly to: Director of School Program.
CORPORATE RESPONSIBILITIES
- Fosters positive interpersonal relationships with the staff, residents, visitors, outside agencies, government officials and related professional associations.

JOB RESPONSIBILITIES
The Team Coordinator provides effective leadership to the team.
- Contributes to an environment conducive to positive learning.
- Plans and implements therapeutic activities that promote optimal functioning, the development of prosocial/interpersonal skills, life skills and adaptive coping skills.
- Develops, implements, monitors and revises both group and individual treatment and academic programs.
- Acts as a liaison between the host school administrators/teachers and the Youthdale program.
- Facilitates treatment and academic discussions in team meetings.
- Works in partnership with team members to develop Individual Education Plans (IEP’s) and Plans of Care (POC’s) for students.
- Schedules POC meeting, parent teacher meetings, meetings with residential and/or CIFT, and ensures that appropriate people are invited.
- Communicates with parents/guardians, residential staff, CIFT and host school administration.
- Provides updates to Director of School Program.
- Attends therapeutic and academic meetings and facilitate group meetings.
- Communicates with Board of Education Teacher(s) and Section 23 administrators. Follows Section 23 guidelines for registering and demitting students. Facilitates the transition planning of students with Section 23 partners.
- Works collaboratively with The Board of Education to ensure seamless coordination of care.
- Keeps detailed records, documents daily progress, evaluates goals in logs, and follows agency guidelines for documentation of incident reports and serious occurrence reporting.
- Organizes, distributes and collects all consent/permission forms and ensure client intake, admission and ongoing documentation is kept up to date.
- Manages the classroom budget, orders supplies and equipment.
- Other duties as assigned.
