Description:

TCP Homecare provides innovative healthcare and pharmaceutical services to meet the requirements of a new emerging healthcare environment in Ireland.
The role of the Dedicated Nurse involves supporting the introduction and management of patients on treatment for Parkinson’s disease whilst providing high quality clinical care.

KEY QUALIFICATIONS & COMPETENCIES:

RGN with NMBI registration
High degree of self-motivation

Ability to form professional relationships with ease

Responsibilities:

  • Liaising with the referring units regarding individual Patient progress and feedback
  • Working within Hospital Units in the key referring centres to pre-educate, on-board/titrate patients onto treatment and to facilitate the transition of Patients from Hospital to the Home Support Service.
  • Provide clinical review, oversight, assessment and advise to our nursing team supporting the designated services.
  • Training patients and/or their carers to facilitate self- administration where relevant.
  • Education of patients/families and colleagues in relation to relevant disease areas
  • Timely electronic reporting at each visit.
  • Maintenance of accurate and detailed records as agreed for each patient.
  • To liaise closely with all other healthcare personnel associated with services ensuring good lines of communication with the hospitals, consultants and other clinical personnel.
  • Assess specific changes in the patient’s condition as a result of new or on-going treatments and take immediate appropriate action.
  • Recognise and report immediately any untoward events on a patient visit. Report all adverse events and product complaints promptly in line with Pharmacovigilance procedures.
  • Implement all TCP Homecare operating procedures and protocols relating to the care and safety of a patient and yourself.
  • Ensure a safe clinical environment for patients and yourself by following all procedures and guidelines.
  • Attend relevant meetings with company and/or clients to provide oversight and details of the service and facilitate meticulous feedback.
  • Participate in approved data collection, clinical studies and trials as requested by the client.
  • Keep abreast of current trends and developments in professional matters and in clinical care. Ensure that clinical skills are maintained.
  • Provide support and advice to patient family member’s clients and multi-disciplinary team.
  • Responsible for the administration of daily/weekly/monthly documentation for departmental reviews.
  • Maintain customer confidentiality at all times.
  • Assist in ensuring that continuing quality systems are implemented and maintained within the TCP Group.
  • Represents self in a professional and ethical manner at all times, complying with all applicable company policy and procedures.
  • Provision of overnight services/long distance driving where required as role is nationwide coverage.
  • To provide support as part of the nurse team during the hours of Monday- Friday 9am-5pm
  • To respond and resolve any problems resulting from a helpline call
  • To inform relevant HCPs regarding a helpline call regarding content and if further follow up required
  • To feedback any helpline calls to nurse team colleagues
  • To document helpline calls in patient’s notes
  • To accurately report all Pharmacovigilance and Product complaint events
  • Participate in appropriate meetings with other departments to communicate necessary information, and present innovative problem solving solutions to create a best practice service.
  • Assist in ensuring that continuing quality systems are implemented and maintained within TCP Group.
  • Represent self in a professional and ethical manner at all times, complying with all applicable company policy and procedures while adhering to the IPHA code of practice and Nursing and Midwifery Board code of conduct.
  • To demonstrate a commitment to the identification of risk, reporting all incidents and near misses.
  • Recognise and report immediately any untoward events on a patient visit. Report all adverse events and product complaints promptly in line with Pharmacovigilance procedures.To adhere to and follow all policies and procedures in regard to decontamination of equipment and principles of infection control.
