Description:
The Deputy Service Manager will provide support to the Service Manager in providing leadership and management for the delivery and provision of the NHS Health Improvement Practitioner Service across Bolton. Health Improvement Practitioners (HIP) are a key part of the local health improvement workforce, tackling health inequalities and supporting the agenda to increase life expectancy of the Bolton population. HIPs are based within GP surgeries across the Borough and work alongside primary care colleagues. This post will be expected to support and provide a comprehensive reporting function, developing support systems to enhance service efficiencies and support performance management.
The role will facilitate a robust evaluation function for the service, subsequently monitoring the outcomes and produce analytical reports as required. An essential part of this remit is to work strategically across the local health & social care economy, collaborating and negotiating with public health, primary care and social care. This collaboration will ensure the service is meeting exacting standards of service delivery and there will be successful progression of ad hoc projects and pathways. The role will require an effective planner with exceptional organisational skills.
The post holder will work autonomously across a range of projects and across organisational boundaries to support a dynamic and progressive HIP Service. As part of the overall duties of the post this will involve generic HIP duties and tasks. This includes managing their own case load of patients, and providing a comprehensive behaviour change service for targeted groups of patients within primary care
