Co-ordinate daily workload on apple tree ward, contribute for the 5 day week cover of stroke coordinator on site facilitating the stroke pathway. Deliver TIA and Stroke care based on principles that are research and evidence based and which enhance the quality of patient care. Initiate subsequent referrals to the appropriate Multidisciplinary Team (MDT) and other appropriate referrals. In theabsence of the stroke ward manager, support the stroke unit nursing team to facilitate patient safety.
Daily contact with Hyperacute Stroke Units at Peterborough and Addenbrookes to ensure patient flow across the stroke pathway and timely /adequate repatriation of patients Assess patients with TIA/stroke liaising with stroke consultant facilitating admission to stroke uni
