Description:
Registered Nurse Professional knowledge acquired through degree, Assess, plan, implement and evaluate care packages for patients on their caseload which can include the analysis of clinical, social and family situations. To exercise accountability for patient care, as set out in the NMC Code of Professional Conduct and act within the policies of Northumbria Healthcare Foundation Trust. To support the Team Lead and in their absence be responsible for the planning and delivery of District Nursing services within a designated area, prioritising clinical need to provide holistic care. To be accountable for own practice working without direct supervision.
To act as an advocate to empower individuals, their carers and their families to influence and use available resources. To carry out risk assessment and management of the physical environment to safeguard patients, staff and carers Promote effective use of Trust policy regarding infection control within the nursing team, the clinical setting and patients homes. Make referrals to other disciplines or agencies
