Herts Valleys Post Hospital Review Team – you will support the Team Manager to lead and manage a busy Integrated Discharge Team supporting those who have been discharged from hospital with a focus on enablement and person-centred working.
We provide a seven-day service to the people of West Hertfordshire (Watford, Three Rivers & Hertsmere areas). You will support the Team Manager to maintain patient flow the Discharge to Assess Pathways by ensuring that the team complete timely and effective social care assessment both in the community setting and the Discharge to Assess beds based in residential units.
About the role
You will ensure that the team maintain a person-centred and strengths-based approach to improve the outcomes for those people who require care and support following their hospital discharge. You will deputise for the Team Manager at system-wide, and other, meetings to ensure the team provides an effective and robust service. You will develop positive working relationships with local system partners, including care providers and the Hospital Trust.
To improve outcomes for service users, the team support a 7-day service. The successful candidate will therefore be asked to work on consecutive Saturdays and Sundays once every eight weeks and will receive days off during the weeks before and after covering a weekend. The successful candidate will be based at Apsley in Hemel Hempstead, but will be able to work flexibly on some days. The successful applicants will be able to evidence sound knowledge of safeguarding practice, working with people with dementia, the personalisation agenda, and Individual Budgets.
You will keep the user and any future potential users at the centre of all activity and address user needs within the legislative framework working to an enabling and ‘home first’ ethos wherever possible.

Key responsibilities of the Deputy Team Manager posts include:

  • Establishing day-to-day practice of the Team, ensuring compliance to relevant policies and procedures and an effective performance management approach.
  • Managing the practice and performance of the team to ensure the timely and effective completion of assessments and reviews and also the exit plan from the discharge to assess pathways
  • To inspire, lead, manage and nurture a team of professionals (Social Workers, Occupational Therapists, home finders and Community Care Officers) that can operate innovatively and creatively in order to meet the needs of Hertfordshire citizens.
  • Building positive relationships with key partners delivering.
  • Effective management of resources and budgets and assurance of quality.
  • Act as the Deputy lead manager in complex / high profile safeguarding and or serious concerns processes.

About you

Essential:

  • You must be a registered Social Worker or Occupational Therapist with experience of managing people and performance
  • You will have experience of leading a busy team and managing change.
  • You will be a skilled communicator
  • You will have supervisory experience.

Desirable:

  • You will be comfortable managing competing demands.
  • You will be able to work flexibly.
