Are you an experienced healthcare support worker with a level 4 certificate in Health Care Studies? Do you want to be part of an exciting new project?
If so, we have a unique opportunity for a full-time (37.5 hours) Diabetes Assistant Practitioner to join the Community Diabetes Team in Pembrokeshire for a 2 year fixed post.
The post holder will be an integral part of the North Pembrokeshire Diabetes cluster project which is aimed at improving access to diabetes surveillance, care and education for those who are housebound/and or not able to access primary care interventions in our locality. The project aims to improve patient outcomes, demonstrate a reduction in lower limb amputations, a reduction in foot ulcers, and actively engage with cohorts experiencing homelessness or housing insecurity.
The project will specifically aim to improve NICE recommended care processes (per National Diabetic Audit) in North Pembrokeshire. The Diabetes Assistant Practitioner will work with the CNS team and primary and community colleagues to expedite proactive foot checks and foot health surveillance, phlebotomy, recording of vital signs, weight checks, perform ankle-brachial pressure index (ABPI) testing, deliver smoking cessation advice, diabetic education, and will liaise with diabetic podiatry, vascular and retinopathy services.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
