Registered Dietitian (RD) or Registered Nurse (RN)

One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services.
We are currently seeking a contract 1 FTE Registered Dietitian (RD) or Registered Nurse (RN who as a valuable member of the Community Health Centre team will provide individual and group diabetes education programs and work collaboratively within an interprofessional model of care team.
The position has a competitive salary, the Health Care of Ontario Pension Plan and an excellent group health benefit package.

QUALIFICATIONS:

  • Registered Dietitian, with current registration in good standing with The College of Dietitians of Ontario OR Registered Nurse, with current registration in good standing with The College of Nurses of Ontario.
  • 3 years of experience in Diabetes Care of adults living with diabetes or newly diagnosed is required.
  • Certified Diabetes Educator designation is an asset.
  • Experience in supporting individuals with complex chronic diseases with self-management of diabetes required.
  • Experience with adult learning principles and motivational interviewing training is an asset.
  • Excellent interpersonal, communication, group facilitation and teaching skills.
  • Ability to work both independently and collaboratively with an interprofessional team.
  • Awareness of and sensitivity to health issues particularly within populations who face barriers to accessing care (under housed, living with mental health challenges or diagnoses, current or past substance use, complex health histories, 2SLGBTQIA+, newcomers, non-insured).
  • Knowledge and proficiency in MS Office (MS Excel/MS Word) and electronic medical record system experience.
  • Ability to work flexible hours including evenings and weekends may be required.
  • Valid driver’s license, and ability to travel to alternate site once every 2 months.
    Eligible for the Community Commitment Program for Nurses (CCPN) that provides $25,000 in grant funding to eligible nurses in exchange for a 2 year commitment. Click here for eligibility criteria.
