Description:

Patient and Clinical Care Be actively involved in the provision of health education and self-management advice to patients, relatives and carers. This includes signposting patients to personalised care colleagues and local community and voluntary sector services. Supporting individuals with all aspects of care including daily living, providing person-centred care and promoting health and independence through awareness raising and care navigation to other professionals as appropriate. Demonstrate good understanding of principles of consent and ensure valid consent is obtained prior to undertaking nursing and care procedures.
Assist in the implementation of appropriate interventions to meet the specific physical, emotional and psychological, social, cultural and spiritual needs of individuals and carers. To undertake initial administrative tasks for people attending diabetic clinics To undertake the following for patients: o Blood Glucose monitoring/Link Nurse training o Blood pressure o Continuous Blood glucose monitors (CGMS) o Pump/ blood glucose meter downloads o Height/weight/waist circumference o Foot checks Undertake simple nursing procedures under direction of the lead nurse. Such duties to include: Ensure adequate supplies for Diabetes clinics e.g. meters, blood glucose strips, sensors etc., are available for clinics.
Inform admin staff when an order is required. Ensure clinic rooms and equipment are made ready prior to start of clinics and left tidy and safe at end of clinic. To input and retrieve data/information from computer as required. To ensure patients feel safe and reassured about the any Diabetes procedure.
In accordance with delegated tasks, organise own workload Be familiar with the use of equipment, especially in relation to moving and handling patients. Assist in the assessment of and contribute to the management of risk within the environment where care is being administered. Give lifestyle advice to patients (including diet, weight and exercise) making any necessary referrals to local support services as required. Undertake appropriate investigation, clinical observations and interventions following agreed guidelines for patients with Diabetes Ensure the privacy, dignity and safety of individuals is maintained at all times.
Recognise issues relating to safeguarding vulnerable children and adults and report any problems or raise concerns to the appropriate registered care professionals. Performance & Quality Develop skills in relation to coaching/teaching individuals/carers/other staff Assist with the implementation and monitoring of clinical standards and outcomes. Demonstrate the ability to recognise changing priorities seeking advice and guidance from the registered nurse or duty doctor as appropriate Develop a working knowledge of providers resources and referral systems to ensure individuals needs are met Enhance own performance through continuous professional development, imparting own knowledge and behaviours to meet the needs of the service parameters of practice. Administration To maintain accurate records of all patient consultations and related work carried out at each clinical session, including computerised data collection.
To contribute to the development of service standards. Resources Make best use of time and resources in order to maximise the benefit to both the patient and organisation. In this way the post holder will support the organisation in delivering value for money. To be responsible for monitoring stock levels in own service area and request new equipment as appropriate.
Knowledge and Skills Supervision Actively contribute own development and learning through supervision and reflection. Communication and Relationships To work effectively with other staff and members of the community (e.g. District Nurses, care staff, carers and relatives) in order to implement patient care plans. To ensure good communications are maintained at all times to achieve the desired outcomes.
To liaise with administration staff around appointments and communication with patients and professionals. To establish and maintain positive interpersonal relationships with other staff which are characterised by open communication, trust and respect. To ensure effective communication of sensitive and confidential information to patients, relatives and carers where there maybe barriers to understanding, which requires interpersonal and negotiating skills. To demonstrate empathy with clients, carers, families and colleagues ensuring that effective communication is achieved particularly when barriers to understanding exist.
To deal with patient expectations and needs sensitively and persuasively.
