Description:

You will be a registered nurse with a first level degree, have current and demonstrable experience of working within a diabetes specialist care team in a Band 6 Diabetes Specialist Nurse role, with a university accredited diabetes qualification. You will also be a non-medical prescriber or be willing to undertake the qualification. The post-holder will facilitate a smooth journey from admission to discharge for the patient with Diabetes admitted to a ward within Bradford Teaching Hospitals. The post-holder will have responsibility for a defined caseload of and will provide an effective and high quality patient-focused service that is sensitive to the needs of users and providers.
Provide clinical, social and psychological support to their patients/service-users using excellent communication skills, education and health promotion to enable them to reach their optimum level of independence and health. Facilitate in the development of the existing diabetes service, in line with local and National Guidance. Develop close working relationships with all members of the Diabetes integrated multidisciplinary team, based in both secondary and primary care
