Description:

JOB SUMMARY:

Under the Sioux Lookout Area Primary Care Team (SLAPCT), the Dietitian will work with patients, caregivers and community resources to provide assessment, consultation and development of treatment plans. The Dietitian is a key part of the patient circle of care and will work to their full scope of practice. Responsibilities will include the provision of safe and competent care through the application of professional knowledge; technical aspects of professional practice and the demonstration of compassion, professionalism and respect. The Dietitian will provide assessments, consultation, development of treatment plans and seamless system navigation. The Dietitian will work in both the clinic environment and communities providing home visits and interventions within patients’ homes.
The Dietitian is directly accountable to the POD Manager.

Qualifications:

  • Persons of First Nations ancestry will be given preference (OHRC, Part II, Special Employment)
  • Bachelor’s Degree in Applied Nutrition, Home Economics or Human Ecology
  • Registered in good standing as a Member of the College of Dietitians of Ontario
  • Certified Diabetes Educator or College Diabetes Education Certificate or be eligible to write and successfully pass the Certification exam and/or completed College Diabetes Education Certificate
  • Ability to travel to remote locations via various modes of travel – automobile, small air plane travel, etc.
  • The ability to perform the requirements of the position on a regular basis.

Roles and Responsibilities:

  • Liaise with frontline workers, health care providers and other service workers in communities to assist in the facilitation for the delivery of Primary Care services
  • Assisting with the planning and coordinating of travel for clients and Allied Health Team
  • Assisting with the planning and coordinating of the activities and schedules of PCT Allied Health Providers/ visiting consultants/specialty clinics that are organized through the Primary Care Team
  • Assistance to Consultant’s when on-site and off-site
  • Completing intake with the client and family (if applicable) and ensure the referral and supporting documents are forwarded to the appropriate health professional
  • Liaise with clients/families, Allied Health Providers and, as directed, action concerns and requests appropriately
  • Entering of data/visit information into the Electronic Health Record
  • Perform routine clerical tasks such as copying, faxing, filing, general typing
  • Maintain privacy and confidentiality in the handling of sensitive/confidential material strictly in confidence in accordance with SLFNHA privacy policy guidelines
  • Participate in multi-disciplinary health team activities/case conferences
  • Participate as part of the multi-disciplinary team on clinic days
  • Compile daily, monthly, annual Summary Reports
  • Provide follow-up support & education to clients and families
  • Assisting in other administrative coverage and Telehealth tasks as directed
  • Other duties as assigned by the Primary Care Management Team
